Around seven in ten leg ulcers are venous, caused by the same leaking vein valves that produce varicose veins, only further down the road. A wound near the ankle that has not healed in two weeks deserves a proper vascular assessment, not just repeated dressings, because the dressing treats the surface while the underlying vein pressure keeps the wound open. The first step is always to check the arteries too, with a simple ankle pressure test, before any firm compression is applied.
The EVRA trial (published in the New England Journal of Medicine, 2018) compared early endovenous ablation of the faulty vein with compression alone in venous leg ulcers. The early-treatment group healed faster and stayed healed longer, with fewer ulcers recurring. The practical message: a venous leg ulcer isn’t only a wound to be dressed, it’s a vein problem to be fixed. And before any compression goes on, an ABPI (ankle–brachial pressure index) is measured to make sure the arteries can tolerate it, a safety check we do on the same visit. The clinic is at 46–48 Wood Street, Kingston upon Thames, KT1 1UW. From Teddington it is around 10 minutes by car over Kingston Bridge via the A313 and High Street, and Hampton Wick is closer still. By rail, Teddington connects to Kingston in a few minutes, then it’s a short walk to Wood Street. Parking is available at the nearby Bentall Centre and Sopwith Way car parks.

Your history, examination, duplex ultrasound and, where appropriate, your treatment are carried out by a consultant on the GMC Specialist Register for Vascular Surgery. For leg-ulcer and circulation patients an ABPI is measured before any compression is applied. Same-day scanning means most patients with straightforward disease leave with a written plan after one visit; where deep venous or arterial disease is suspected, a dedicated scan may still be needed and will be arranged. Initial consultation with duplex ultrasound is £365 (one leg) or £520 (both legs) self-pay, with no GP referral required.

A wound near the ankle that has not healed in about two weeks should have a vascular assessment, because most leg ulcers are venous and stay open until the underlying vein pressure is treated. We assess the veins and arteries (including an ABPI) and build a healing plan, with early vein treatment where it will help.
ABPI (ankle–brachial pressure index) compares blood pressure at the ankle and arm to check the arteries can tolerate compression. Firm compression on a leg with poor arterial supply can be harmful, so we always measure ABPI first, on the same visit.
