Emergency

When is chest pain an emergency?

Call 999 if:
  • you get sudden pain or discomfort in your chest that does not go away
  • the pain feels like squeezing or pressure inside your chest, burning or indigestion
  • you get pain that spreads to your left or right arm, or your neck, jaw, stomach or back
  • you have chest pain and you feel sweaty, sick, light headed or short of breath

You could be having a heart attack. Call 999 straight away. Do not drive yourself to A&E.

If you had chest pain in the last 3 days that may have come from your heart and you have not been checked, NICE guidance says you need a same day hospital assessment. Call NHS 111 or your GP today rather than waiting for a clinic appointment.

Who it is for

When should you see a cardiologist about chest pain?

See a doctor if you have chest pain that comes and goes, or chest pain that goes away quickly but you are still worried. A cardiologist can work out whether it is coming from your heart.

Pain on exertion

Chest tightness or discomfort that comes on with exercise or stress and eases with rest, which can be a sign of angina.

Pain that keeps coming back

Chest pain or discomfort that comes and goes and has not been explained.

Risk factors

Chest pain with high blood pressure, raised cholesterol, diabetes, smoking or heart disease in the family.

Peace of mind

Chest pain that has gone but still worries you.

Causes

What else can cause chest pain?

Most chest pain is not caused by the heart. Common causes include:

The cardiologist’s job is to find out whether your heart is the cause, and to tell you clearly when it is not.

Tests

How is chest pain investigated?

At KONCEPT® in Kingston, assessment of chest pain starts with your history, an examination and a 12 lead ECG. If the cardiologist thinks your pain could be angina, NICE recommends a CT coronary angiogram, a scan of the arteries that supply your heart. This is done at a hospital or imaging centre, and he refers you to the one you choose.

Blood tests for cholesterol and blood sugar, and an echocardiogram, may also help.

Insurance

Can I use my health insurance for a chest pain assessment?

Many private medical insurance policies cover a chest pain assessment when it is needed to investigate symptoms, if your policy includes outpatient cover. For your insurer to pay, the clinic and the cardiologist usually both need to be recognised by that insurer, and most insurers ask for a GP or open referral and a pre authorisation number first.

Please call 020 8129 1011 with your insurer and policy number before you book, so we can confirm your cover in writing. If you pay for yourself, no GP referral is needed.

Your cardiologist

Who will you see?

You see Dr Ameer Al-Bassam, a specialist and interventional cardiologist on the GMC Specialist Register for cardiology, who also works in the NHS at Chelsea and Westminster Hospital NHS Foundation Trust.

Questions

Chest pain questions

Should I book an appointment if I have chest pain now?

No. If you have chest pain now that does not go away, call 999. A clinic appointment is for chest pain that comes and goes, or has gone but still worries you.

How do I know if chest pain is my heart?

You cannot always tell from the pain alone. Pain that feels like squeezing or pressure, spreads to your arm, neck, jaw or back, or comes with sweating, sickness or breathlessness needs a 999 call. Pain that comes on with exercise and eases with rest can be angina and should be checked.

Can anxiety cause chest pain?

Yes. Anxiety and panic attacks can cause chest pain, often with a fast heartbeat, sweating and dizziness. If you are not sure, it is still worth having your heart checked.

Do you do CT scans of the heart at KONCEPT®?

No. If you need a CT coronary angiogram, the cardiologist refers you to a hospital or imaging centre of your choice.

Do I need a GP referral to see the cardiologist?

No, not if you are paying for yourself. If you are using health insurance, your insurer will usually ask for a referral and a pre authorisation number first.
