Sexual symptoms in men rarely have a single cause. Low testosterone, raised prolactin, thyroid dysfunction, undiagnosed diabetes or pre-diabetes, raised lipids and vascular disease, prostate enlargement, medications, sleep and stress can all contribute in different combinations. A focused testosterone-only panel can miss the diagnosis. The result is interpreted by the doctor in context. Each result is interpreted by the doctor against the appropriate reference range for you. Testosterone is most accurate from a morning sample (7am to 10am). PSA should not be tested within 48 hours of ejaculation, vigorous exercise (cycling), digital rectal examination or recent urinary infection, as all can falsely raise the result.

What happens after your results

Most cases respond to a combination of lifestyle measures (sleep, weight, alcohol, exercise), treating reversible causes (low vitamin D, low iron, suboptimal blood sugar) and short-course or longer-term treatment where appropriate. Who is this panel for?

What does the panel measure?

The male hormone axis (total testosterone, SHBG, calculated free testosterone, Free Androgen Index, prolactin), thyroid function (TSH, Free T4), blood sugar (glucose, HbA1c), full lipid profile, and PSA. Eleven blood markers in total.

Why is PSA in a sexual-health panel?

PSA is included as a screening adjunct because some men presenting with libido or erectile symptoms also have prostate symptoms or are at the age where a baseline PSA is reasonable to obtain alongside the hormone and metabolic markers. PSA is not a definitive prostate cancer test; raised PSA can also reflect benign prostate enlargement, prostatitis, recent ejaculation or a urinary infection. The doctor interprets the result in context. For a dedicated prostate cancer risk score, the Stockholm3 panel is the better choice.

What's the link between erectile difficulty and heart disease?

ED is recognised by NICE CKS and the wider clinical literature as a possible early indicator of cardiovascular disease, particularly in men under 60. The penile arteries are smaller than the coronary arteries, so vascular problems can become apparent there first. This is one reason the Sexual Health panel includes lipid and HbA1c markers alongside the hormone profile, and why the reviewing doctor reads the panel as a whole rather than as separate results.

Could low testosterone be linked to diabetes?

Type 2 diabetes is associated with lower testosterone in some men, and low testosterone can in turn affect insulin sensitivity. HbA1c and testosterone are both included in this panel so the doctor can see whether the picture is hormonal, metabolic or both. Addressing diabetes risk often improves testosterone naturally; the reverse can also be true.

When should I see a urologist?

The reviewing doctor will recommend a private or NHS urology referral if PSA is persistently raised after a repeat test, if there are urinary or prostate symptoms (poor stream, frequency, nocturia, blood), or if the clinical picture warrants it. Most ED in men under 60 is initially worked up in primary care with a focus on cardiovascular and metabolic causes; urology referral typically comes when those have been addressed.

When should the blood test be done?

Morning, ideally between 7am and 10am. Fast for 10 to 12 hours (water only) for accurate glucose and lipids. Avoid ejaculation, vigorous exercise (cycling) and digital rectal examination for 48 hours before to keep PSA reliable.

What if my PSA comes back raised?

Raised PSA in isolation is not a diagnosis. The doctor will repeat the PSA after addressing reversible causes (recent ejaculation, exercise, infection) and will recommend onward urology referral (NHS or private) where appropriate. The Stockholm3 panel can also help guide whether MRI or biopsy is needed.

What if my testosterone comes back low?

A confirmatory second early-morning sample is recommended per NICE CKS guidance, alongside a discussion of contributing factors. If both samples confirm low testosterone, specialist referral is offered. KONCEPT® does not initiate long-term testosterone replacement therapy from a single panel result.
