A heart blood test is a focused cardio-metabolic baseline. It identifies pre-diabetes and lipid patterns early, when lifestyle change still has the largest effect, and informs whether further work-up (such as the Advanced Cardiovascular Risk panel with Lp(a) and ApoB) is appropriate.

This panel covers the routine blood markers that drive both: HbA1c and glucose for sugar control, the full lipid profile for cholesterol, and liver enzymes (ALT, AST) for non-alcoholic fatty liver disease, which is closely linked to insulin resistance. Fasting insulin is added when there is a clinical indication. Glycated haemoglobin

The three-month average of blood sugar. The most sensitive routine screen for pre-diabetes and type 2 diabetes, per NICE CKS guidance. Unlike fasting glucose, HbA1c does not need fasting.

Fasting glucose

Glucose

A morning fasted glucose alongside HbA1c picks up acute changes and helps confirm an impaired fasting glucose pattern. The two are read together.

ALT and AST screen for non-alcoholic fatty liver disease (NAFLD), now better called metabolic dysfunction-associated steatotic liver disease (MASLD), which sits squarely in the metabolic risk picture and is the most common cause of mildly raised ALT in middle-aged men.

Fasting insulin (where indicated)

Insulin

Added where insulin resistance is clinically suspected. Read alongside fasting glucose to look at HOMA-IR, an index of insulin resistance. Not added routinely.

Each result is interpreted by the doctor against the appropriate reference range for you. NICE risk calculators (QRISK) combine blood markers with age, blood pressure and lifestyle for a 10-year cardiovascular risk estimate; the doctor can apply this to your results.

How the panel maps onto your risk picture

  • Pre-diabetes range: HbA1c 42 to 47 mmol/mol (6.0 to 6.4%) per NICE CKS. Picked up by HbA1c before symptoms appear; lifestyle change has the largest effect at this stage.
  • Type 2 diabetes: HbA1c at or above 48 mmol/mol (6.5%) on two readings, or a single reading with symptoms.
  • Atherogenic lipid pattern: raised LDL and non-HDL with low HDL and raised triglycerides; insulin resistance commonly underlies this combination.
  • Suspected metabolic-associated fatty liver (MASLD): mildly raised ALT in the context of central adiposity or pre-diabetes. The doctor will explain whether further imaging or specialist referral is appropriate.
  • When the panel is reassuring: normal HbA1c, normal lipid profile and normal liver enzymes give a clean cardio-metabolic baseline. Repeat timing depends on your age, family history and lifestyle.

What if my cholesterol comes back high?

Raised cholesterol is interpreted in the context of your QRISK score. Most cases are addressed first with lifestyle change and reassessed; medication (typically a statin) is considered where overall risk justifies it, per NICE CKS guidance. The reviewing doctor will outline the options in the outcome note.

What if my HbA1c comes back in the pre-diabetes range?

Pre-diabetes (HbA1c 42 to 47 mmol/mol) is a strong signal for lifestyle intervention. The NHS Diabetes Prevention Programme and lifestyle change can return many men to normal HbA1c. Repeat HbA1c in 6 to 12 months is typically recommended.
